How To Enrol In An Aquilian Benefits Plan?

There is no need to enrol until you are ready to submit a first claim for yourself or an employee. When you wish to enrol, follow this simple 3-step process:

1. Download the Enrolment Form. All our forms are also available on the Forms page.
2. Fill out the form on-screen or print it and complete it by hand. This is your Third-Party Contract of health benefit coverage. Use Appendix A to list the name(s) of your Covered Employees, even if you are the only one.
3. Email the forms to info@aquilian.ca.

You may wish to submit your first claim along with your enrolment application. We will open your account and adjudicate your claim at the same time.
